Responsible for the coordination of patient care in a multidisciplinary clinical setting. Works with various providers and services to ensure the best possible treatment and outcome for patients. Coordinates necessary patient care services with physicians, nurses, lab, radiology, pharmacy, infusion and clinics.

WORK SCHEDULE: Full-time, 40 hours per week, day shift.


COMPENSATION: $53,414"$79,400. Strong benefit package including tuition reduction program, generous leave accrual, retirement and health plan options.



  1. Directs the scheduling of patient appointments with providers and ancillary services.
  2. Follows up on patients treatment, ensuring that patient needs are being met and that they understand the course of their treatment.
  3. Triages patient questions and ensures appropriate coordination and follow through.
  4. Evaluates referrals from physicians and self-referred patients.
  5. Gathers needed medical information such as records, slides and films to assist in the evaluation of patient needs.
  6. Coordinates treatment-planning conferences and presentations.
  7. May develop written protocols for communication to patients regarding treatment plans.
  8. May assist with communicating the treatment plan to patients.
  9. May act as a liaison with other care groups such as social work, financial counseling and nutrition.



Bachelors degree in Nursing required. Demonstrated human relation and effective communication skills are also required.

Experience in multidisciplinary clinics, and/or working with high-acuity, complex patients may be preferred.

This position is not responsible for providing direct patient care.

Current licensure to practice as a Registered Nurse in the State of Utah or equivalency required. *If licensed in a compact state a Utah State license must be obtained within 30 days of hire.

Basic Life Support Health Care Provider card or must obtain one within one month of hire required.



3-5 years experience in Obstetrics/Labor & Delivery/Womens Health, multidisciplinary clinics or working with high-acuity, complex patients.

Spanish speaking

Diabetes educator training

Microsoft Office proficiency
